How to Book Appointment?

1. Click BOOK APPOINTMENT button on our website.

2. You will be directed to our management system powered by JANE.

3. 

   One JANE's booking page, you can browse through our available services and our availability by month.

4. Select your desired service date & time to start booking.

5. 

   You will then be asked to Sign in or Create a Patient Account.

6. Follow JANE's instructions to complete account registration and booking.

7. 

   You will then be prompted to complete an initial intake form and sign consent forms. See above image step 2 Fill Out Intake Form.

8. After completing all required housekeeping steps, your appointment booking is now completed.
